Social Services Assistant

Under direction of Social Services Director, our Social Services Assistant will assist each resident and family in adjusting to placement, illness, and plan of care to attain the highest practicable level of functioning in accordance with current federal and state regulations, and Company policies and procedures. You will assist resident and family wishes regarding treatment and assist with resident and family transition and adjustment to the facility; and participate in resident and family council meetings as requested. You will also support Admissions and assist with discharge planning services.

Job Overview:

  • Assist in developing and implementing policies and procedures for identifying the medically related social and emotional needs of the resident.
  • Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
  • Participate in discharge planning, development and implementation of social care plans and resident assessments.
  • Interview residents/families as necessary and in a private setting.
  • Make routine visits to residents and perform services as necessary.
  • Assist in interpreting social, psychological, and emotional needs of the resident/family to the medical staff, attending physician, and other resident care team members.
  • Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the resident.
  • Perform charting duties as necessary.
  • Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Director.
  • Assist in establishing procedures to be followed in the collection, coding and indexing, and the filing/retrieving of medical records.
  • Collect, assemble, Index, check, and file resident charts and personnel records as may be necessary.
  • Complete the initial Social Service Assessment and the initial patient interview profile to begin discharge planning upon admission
  • Provide information to family and/or patients/residents relating to Medicare, Medicaid, and other financial assistance programs available to the patient/resident
  • Work closely with Case Management to coordinate patient discharges

QUALIFICATIONS & REQUIREMENTS:

  • Must possess at minimum a High School Diploma or equivalent G.E.D
  • Experience in the nursing field. (C.N.A., etc)
  • Possible experience in long term care or skilled nursing facility

Job Type: Full-time

Pay: From $16.50 per hour
