Social Services Facilitator
JOB PURPOSE:
The Social Services Facilitator supports student success by identifying barriers and connecting students and families to appropriate resources and services. This role focuses on improving student mental health, attendance, and engagement through caseload management, coordination of supports for special populations (including 504 and health plan students), and facilitation of Student-Based Team (SBT) processes.
PRIMARY RESPONSIBILITIES:
- Creates a student-centered environment that promotes engagement, well-being, and academic success
- Supports orientation to ensure strong student transitions
- Manages a caseload of Tier 2 and Tier 3 students, including those with 504 plans and health-related needs, ensuring coordination and compliance of services and supports
- Monitors student progress across attendance, behavior, and academic indicators
- Facilitates Student-Based Team (SBT) meetings to develop, implement, and monitor individualized intervention plans
- Serves as a liaison between students, families, staff, and community agencies to coordinate support and services
- Provides short-term support, guidance, and referrals to address social, emotional, and environmental needs
- Provides indirect services on behalf of students, including referrals and consultation/collaboration with parents, teachers, administrators, and stakeholders to promote educational equity, access, and success
- Advocates for student equity and access to a world-class education that leads to high school graduation and post-graduate success
- Maintains accurate documentation and ensures compliance with policies and regulations
- Supports staff in analyzing student performance data, setting rigorous subgroup goals, and developing action plans to improve academic, social/emotional, and post-graduate outcomes while promoting a positive school climate
- Supports school-wide goals related to attendance, enrollment, retention, and student success
- Assists administration with operational duties related to attendance and student support
EDUCATION AND EXPERIENCE REQUIRED:
- Associate’s degree required; Bachelor’s degree preferred in social work, human services, or related field.
- Experience working with case management, at-risk youth, or families preferred.
- Knowledge of community resources and social service systems.
- Strong communication and interpersonal skills.
- Ability to work with diverse populations and maintain confidentiality.
- Proficiency in computer applications and data systems.
- Valid driver’s license and ability to travel.
WORK ENVIRONMENT:
Indoors and outdoors; role may require home visits and community-based meetings.

Pay: $50,000.00 - $60,000.00 per year
