Aspire Physical Recovery Center at Hoover is a premier, state-of-the-art short-term rehabilitation and long-term care facility. We are dedicated to providing exceptional physical, emotional, and social care to our residents. We are seeking a compassionate, dynamic, and organized professional to advocate for our residents, support their families, and lead seamless care transitions.
We welcome applicants with a clinical nursing background (LPN) or a human services background. A dedicated social work license is NOT required for this role.
Key Responsibilities
Advocacy: Serve as the primary advocate for resident rights, preferences, and well-being.- Assessments: Conduct comprehensive psychological and social assessments to develop individualized care plans.
- Discharge Planning: Coordinate safe, efficient care transitions and discharge planning back home or to alternative care settings.
- Support Services: Provide crisis intervention, emotional support, and counseling to residents and their families facing complex medical journeys.
- Collaboration: Partner with nursing, therapy, and medical leadership teams to deliver holistic care.
- Compliance: Maintain meticulous documentation in accordance with state, federal, and facility guidelines.
Qualifications & Skills
Education & Background: We are looking for EITHER of the following qualification paths:- A Licensed Practical Nurse (LPN) looking to transition into a social services and discharge planning role, OR
- A Bachelor’s Degree in Social Work (BSW), psychology, sociology, or a closely related human services field.
- Licensure: Active Alabama LPN license OR applicable human services credentials. A specific social worker license is NOT required.
- Experience: Previous experience in a skilled nursing facility, rehabilitation center, or hospital setting is strongly preferred.
- Interpersonal Skills: Proven ability to deal tactfully, patiently, and positively with residents, family members, staff, and state regulators.
- Communication: Exceptional written and verbal English communication skills.
