Duties
- Conduct thorough patient assessments, including intake interviews and diagnostic evaluations, to determine individual needs and develop tailored care plans.
- Provide clinical counseling, behavioral therapy, and mental health counseling to individuals and groups, utilizing techniques such as cognitive behavioral therapy (CBT), motivational interviewing, and applied behavior analysis (ABA).
- Coordinate with multidisciplinary teams to ensure seamless case management, patient care, and documentation in compliance with HIPAA regulations.
- Assist with crisis intervention efforts, including de-escalation techniques and emergency response procedures for patients experiencing mental health crises or behavioral incidents.
- Collect and maintain accurate medical records, including progress notes, treatment plans, and medical documentation using electronic medical record (EMR) systems.
- Support patients with disabilities or developmental delays through specialized services such as autism spectrum disorder interventions or dementia care strategies.
- Engage in community outreach and home-based services to deliver hospice care, post-acute care, or senior care in various settings including assisted living facilities and inpatient units.
Experience
- Prior experience working in social work, behavioral health, or mental health counseling environments is highly desirable.
- Demonstrated knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and medical records management.
- Experience with crisis management, trauma-informed care (including PTSD care), and working with individuals with disabilities or developmental conditions such as autism or Alzheimer’s disease.
- Familiarity with hospital settings, outpatient clinics, nursing homes, or home & community care environments enhances your ability to deliver effective support.
- Proven ability to handle sensitive situations with professionalism while maintaining confidentiality in accordance with HIPAA standards.
- Strong skills in data collection for clinical research or quality improvement initiatives are a plus.

Pay: $55,000.00 - $62,500.00 per year
