Social Services Worker

Job Summary
We are seeking a compassionate and dedicated Social Services Worker to join our dynamic healthcare and community support team. In this vital role, you will provide comprehensive social services to individuals across diverse settings, including hospitals, outpatient clinics, assisted living facilities, and community programs. Your expertise will help clients navigate complex emotional, social, and medical challenges, ensuring they receive the personalized care and support they need to thrive. Responsibilities

  • Conduct thorough patient assessments to identify social needs, mental health concerns, and eligibility for various programs
  • Develop and implement individualized care plans through clinical counseling, behavioral therapy, and motivational interviewing techniques
  • Provide crisis intervention and immediate support during emergency situations or mental health crises
  • Facilitate individual and group therapy sessions focusing on mental health counseling, addiction counseling, or behavioral health issues
  • Collaborate with healthcare teams to coordinate services such as hospice care, dementia care, or post-acute care for patients with complex needs
  • Collect and document medical records, clinical notes, and case management data in compliance with HIPAA regulations using Electronic Medical Records (EMR) systems
  • Assist with intake procedures and manage referrals to specialized services like special education or autism support programs

Requirements

  • Bachelor’s degree in social work, psychology, counseling or a related field; Master’s degree preferred
  • Proven experience working in hospital settings, inpatient units, nursing homes, or home & community care environments
  • Knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and medical documentation standards
  • Familiarity with behavioral therapy techniques such as cognitive behavioral therapy (CBT) and applied behavior analysis (ABA)
  • Strong skills in crisis management, case management, and diagnostic evaluation for diverse populations including children, seniors, individuals with disabilities or developmental disabilities
  • Ability to work effectively with individuals experiencing PTSD or dementia-related conditions like Alzheimer’s disease
  • Experience utilizing EMR systems and understanding of HIPAA compliance for patient confidentiality
  • Excellent communication skills for intake interviews, patient assessment discussions, and collaboration with multidisciplinary teams

Pay: $65,000.00 - $70,000.00 per year
