Social Work Care Manager / RN Nurse Care Manager

Hybrid: Work From Home and Field-Based 1099 Independent Contractor Position

Pay: $48–$55 per Billable Hour

Minimum Availability Required: Three Full Days Per Week, 24 hours weekly minimum.

ElderCare Connections is seeking an experienced, dependable, and highly organized Social Work Care Manager or Nurse Care Manager to join our growing geriatric care management team. This position may be a strong fit for a nurse, social worker, or experienced care management professional who is confident working independently, understands the needs of older adults and their families, and can manage complex client situations with professionalism, compassion, and follow-through.

We are looking for someone who can step into client care needs with confidence, communicate clearly with families and providers, attend appointments, document thoroughly, coordinate next steps, and ensure tasks are completed without needing constant oversight. The right person for this role will be proactive, reliable, solutions-focused, and comfortable managing the many moving pieces that come with aging care, medical advocacy, family communication, facility coordination, client moves, community referrals, and care transitions.

This is not an occasional, one-day-a-week role. While this is a flexible 1099 independent contractor position paid per billable hour, applicants must have availability at least three full days per week in order to support client continuity, timely follow-up, and consistent care coordination.

Position Overview

The Social Work Care Manager / Nurse Care Manager will provide direct support to older adult clients and their families through client visits, care coordination, documentation, family communication, advocacy, referrals, community partner collaboration, and ongoing support. This is a hybrid role that includes both work-from-home responsibilities and field-based work. Remote work may include documentation, phone calls, emails, family follow-up, provider communication, referral coordination, scheduling support, and other care coordination tasks. Field-based work may include client visits, medical appointments, facility meetings, hospital or rehabilitation visits, home visits, client move coordination, and community partner meetings.

This role requires someone who is comfortable taking ownership of assigned client needs, following through on details, and keeping the care plan moving without relying on constant direction. The ideal candidate will already understand how to navigate complex care situations and will be able to assess needs, communicate concerns, identify next steps, and complete follow-up in a timely and professional manner.

Key Responsibilities

Responsibilities may include:

-Completing client visits in homes, assisted living communities, memory care settings, hospitals, rehabilitation centers, skilled nursing facilities, or other care settings.

- Working from home to complete documentation, phone calls, emails, family communication, referrals, and care coordination tasks.

- Providing timely, thorough, and professional documentation after visits, appointments, calls, messages, and care coordination activities.

- Communicating with family members and designated points of contact regarding client status, concerns, updates, and next steps.

- Coordinating care with physicians, specialists, facility staff, home care agencies, home health providers, hospice teams, therapists, attorneys, financial professionals, and other community partners.

- Attending medical appointments, care plan meetings, discharge planning meetings, facility meetings, and other client-related appointments as needed.

- Helping coordinate client moves, including communication with facilities, families, movers, vendors, home care agencies, and other involved parties.

- Identifying changes in condition, safety concerns, gaps in care, family concerns, and areas requiring follow-up.

- Making appropriate community partner referrals and helping connect clients and families with needed resources.

- Maintaining and strengthening relationships with community partners, referral sources, facilities, providers, and service agencies.

- Responding to client and family phone calls or messages outside of scheduled working hours when needed.- Following through on assigned tasks without repeated prompting.- Keeping ElderCare Connections leadership informed of significant updates, concerns, recommendations, and completed follow-up items.

- Supporting families through complex aging-related decisions in a calm, professional, and compassionate manner.

- Maintaining confidentiality, professionalism, and sound judgment in all client and family interactions.

Schedule and Availability

This position offers flexibility in scheduling; however, applicants must have availability at least three full days per week. The exact days and hours may vary based on client needs, appointments, urgent matters, family communication, client moves, and care coordination demands.

Because geriatric care management often involves time-sensitive communication, family phone calls and messages may occur outside of scheduled working hours. Time spent responding to client or family calls, messages, and care coordination needs is billable and paid in accordance with ElderCare Connections’ billing and documentation standards.

This role is best suited for someone who can offer consistent availability, manage follow-up in a timely manner, and remain responsive when client or family needs arise.

Work Environment

This is a hybrid role that includes both remote work and field-based responsibilities. The contractor will complete some work from home, including documentation, communication, scheduling, and coordination tasks. The contractor will also travel locally to client homes, facilities, hospitals, rehabilitation centers, medical offices, and community partner locations as needed.

Reliable transportation, comfort working independently in the field, and the ability to manage remote communication professionally are required.

Expectations

This role requires strong independence, reliability, and follow-through. The selected contractor must be able to manage multiple client needs, prioritize effectively, document clearly, and communicate professionally.

Applicants must be able to:

- Maintain availability at least three full days per week.

-Travel to facilites and homes in Stafford, Fredericksburg, Spotsylvania, King George, Bowling Green, & Orange VA as required.

- Work effectively from home and independently in the field.

- Complete documentation in a timely and professional manner.

- Follow through on client needs, family communication, referrals, and coordination tasks.

- Work independently without requiring step-by-step direction.

- Respond appropriately to family calls, messages, and time-sensitive client needs, including outside of scheduled hours when necessary.

- Communicate clearly when a concern, barrier, or urgent issue arises.

- Represent ElderCare Connections professionally in the community.

- Build trust with clients, families, providers, and referral partners.

- Manage complex and sometimes messy situations with maturity, calmness, and good judgment.

Ideal Candidate

The ideal candidate is someone who already understands geriatric care, healthcare systems, family dynamics, and the importance of closing the loop. This person should be able to walk into a complex client situation, assess what is happening, determine what needs to be done, communicate effectively, and move the situation forward.

We are looking for someone who is steady, organized, dependable, and comfortable working with older adults who may have dementia, chronic illness, functional decline, limited support, complex family dynamics, or changing care needs.

The ideal candidate will be:

- A licensed Registered nurse, social worker, or experienced care management professional.

- Comfortable working independently and making sound professional judgments.

- Highly reliable with consistent weekly availability.

- Skilled at documentation, communication, and follow-through.

- Able to manage multiple client needs at one time.

- Familiar with assisted living, memory care, skilled nursing, home care, home health, hospice, hospital discharge planning, and community-based aging services.

- Comfortable communicating with families, providers, facilities, and referral partners.

- Comfortable developing and maintaining community relationships.

- Compassionate, professional, resourceful, and calm under pressure.

- Able to recognize when a situation requires escalation or leadership involvement.

Preferred Qualifications

Preferred qualifications include:

- RN, BSN, MSN, LMSW, LCSW, BSW, MSW, or related healthcare/care management background.

- Experience working with older adults and families.

- Experience in care management, case management, discharge planning, home health, hospice, assisted living, skilled nursing, hospital social work, or geriatric services.

- Strong written and verbal communication skills.

- Reliable transportation and willingness to travel throughout the Fredericksburg region and surrounding areas.

- Ability to use email, electronic documentation systems, calendars, and basic technology.

- Professional judgment, discretion, and respect for client confidentiality.

Compensation and Contractor Status

This is a 1099 independent contractor position paid at $48–$55 per billable hour, depending on credentials, experience, and fit for the role. ElderCare Connections offers a strong billable hourly rate that reflects the level of independence, professionalism, follow-through, and clinical/care management judgment required for this position.

Billable time may include client visits, documentation, family communication, provider communication, care coordination, phone calls, messages, referrals, client move coordination, community partner communication, and other approved client-related work. Although this position is paid per billable hour, a minimum availability of three full days per week is required due to the nature of client care, family follow-up, appointments, coordination needs, and time-sensitive situations.
