POSITION SUMMARY

To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patient’s needs/wishes with team members, family members, caregivers, and other providers.

ESSENTIAL FUNCTIONS

• Assess the psychosocial needs of the patient and family members
• Provide therapeutic counseling and emotional support to patients and family members and education on end of life issues
• Contribute to and execute the Plan of Care with family/patient
• Record daily documentation of visits and contact in accordance with Policy
• Provides referrals to community resources
• Performs in-office/non-clinical administrative duties
• Perform other duties as assigned

POSITION QUALIFICATIONS

• Bachelor's of Social Work with 1 year of applicable experience required; Master's degree preferred
• Texas State license required
• Reliable transportation and a valid driver's license.
• Hospice or healthcare experience preferred.
• Proficient skills using a computer are required
