Social Worker

Looking for a Social Worker with nursing facility experience for a busy rehabilitation center to provide case management and discharge planning. Our social service department has two full time positions that work together to support the needs of our patients and residents; the other position provides support and the MDS assessments. Compensation commensurate with degree, certifications and years of experience, LSW preferred but not necessary.

Social Worker responsibilities include these skills:

• Leading patient care conference meetings to develop discharge plans and goals between the patient, their family, and the medical & therapy teams.
• Educating short-stay patients and families how to file health insurance appeals, if they desire to extend their rehabilitation stay.
• Coordinating and providing accurate documentation of all discharge plans.
• Assisting patients and families to connect with medically needed ancillary services, such as dental, optometry, audiology, mental health, etc
• Providing education and assistance to families about facility services, and resources in the community for post discharge care.
• Participating in daily management meetings and facilitate weekly utilization review meetings together with Nursing and Therapy Mgrs.

Requirements:

• Previous SNF experience essential
• MDS assessment and Point Click Care skills and experience essential
• Previous acute care experience beneficial
• LSW preferred but not necessary if have SNF experience instead
