Job Summary

We are seeking a compassionate and dedicated Social Worker to join our Adult Medical Day Care team. In this role, you will provide psychosocial support, care coordination, and case management services to elderly and medically complex participants. You will play a critical role in enhancing participants’ quality of life by addressing needs while supporting families and caregivers. This position offers the opportunity to make a meaningful impact in a community-based healthcare setting through collaborative, participant-centered care.

Responsibilities

  • Conduct comprehensive psychosocial assessments upon admission and at regular intervals, in accordance with NJ DOH regulations
  • Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team
  • Provide individual and group counseling to support emotional well-being, adjustment, and coping strategies
  • Assist participants and families with care planning, discharge planning, and linkage to community resources
  • Provide support for participants with cognitive impairments, including dementia and Alzheimer’s disease
  • Address behavioral concerns and provide intervention strategies to maintain a safe and supportive environment
  • Facilitate communication between participants, families, and healthcare providers
  • Maintain accurate, timely, and HIPAA-compliant documentation
  • Assist with crisis intervention and ensure appropriate follow-up and documentation
  • Participate in quality improvement initiatives and compliance with state regulations

Skills & Qualifications

  • Bachelor’s or Master’s degree in Social Work (BSW or MSW required; LSW preferred for NJ)
  • Experience working with geriatric populations, adult day care, or long-term care settings preferred
  • Ability to conduct assessments, develop care plans, and manage caseloads effectively
  • Excellent interpersonal and communication skills, with ability to work with diverse populations
  • Strong organizational skills and ability to collaborate within an interdisciplinary team
  • Knowledge of community resources and benefits available to seniors

  • Regular coordination with nursing, activities, and administration
  • Focus on quality of life, social engagement, and family support rather than purely clinical therapy
  • Structured documentation tied to state survey and compliance requirements

Pay: $56,478.72 - $73,000.00 per year
