Location: Cumming, GA
Shift: 8-hour Evening (12:00 PM – 8:30 PM), Monday–Friday
Duration: 13 Weeks (04/13/2026 – 07/11/2026)
Guaranteed Hours: 40 hours/week
On-Call: None

Duties

• Conduct psychosocial assessments and develop care plans
• Provide crisis intervention and emotional support to patients and families
• Coordinate discharge planning and community resource referrals
• Collaborate with physicians, nurses, and case managers
• Ensure compliance with hospital policies and regulatory standards
• Maintain accurate and timely documentation

Experience

• Minimum 2 years of hospital-based social work experience
• Active LCSW license in the state of Georgia
• BLS certification required
• Strong communication, organizational, and interpersonal skills

Job Types: Full-time, Contract, Temporary

Pay: $1,700.00 - $1,800.00 per hour
