Section 1: Position Summary

The Social Worker is responsible for providing direct social services to individuals, families, and groups within the community and/or designated service area. This position involves conducting comprehensive assessments of client needs, developing and implementing individualized care plans, and connecting clients with appropriate community resources and support systems. The Social Worker plays a vital role in supporting vulnerable populations — including those affected by poverty, mental illness, substance use disorders, trauma, or domestic violence — by advocating for their rights and facilitating access to services that promote their health, safety, and overall well-being. This role requires a high degree of professional judgment, ethical conduct, and a commitment to person-centered, culturally competent practice.

Section 2: Essential Duties and Responsibilities

The following duties represent the primary responsibilities of this position. Other duties may be assigned as needed.

  • Conduct comprehensive psychosocial assessments to identify client needs, strengths, and risks.
  • Develop, implement, and monitor individualized service plans in collaboration with clients and multidisciplinary teams.
  • Provide individual, family, and group counseling and crisis intervention services.
  • Connect clients with community resources, referrals, and support services including housing, food assistance, mental health services, substance abuse treatment, and more.
  • Maintain accurate, timely, and confidential client records and case notes in compliance with agency policies and applicable federal, state, and local regulations.
  • Advocate for clients' rights and access to appropriate services across all systems of care.
  • Collaborate with healthcare providers, schools, courts, and other agencies to coordinate comprehensive, integrated care.
  • Participate in case conferences, interdisciplinary team meetings, and regularly scheduled supervisory sessions.
  • Comply with mandatory reporting requirements and uphold ethical standards in accordance with the NASW Code of Ethics.
  • Other duties as assigned.

Section 3: Qualifications

Education

  • Bachelor's degree in Social Work (BSW) required; Master's degree in Social Work (MSW) preferred.

Licensure

  • Current licensure as required by state law (LSW, LCSW, or equivalent) preferred.
  • Must maintain licensure in good standing throughout the duration of employment.

Experience

  • Minimum 1–2 years of experience in social work, human services, or a related field required; may include supervised field placement hours.
  • Experience with [target population — e.g., children and families, elderly individuals, mental health populations, etc.] preferred.

Skills and Competencies

  • Strong assessment, counseling, and case management skills with demonstrated ability to manage complex caseloads.
  • Excellent written and verbal communication skills, including documentation and report writing.
  • Proficiency in electronic health records (EHR) or case management software systems.
  • Ability to work effectively both independently and as part of a multidisciplinary team.
  • Cultural competence and demonstrated commitment to diversity, equity, and inclusion in service delivery.
  • Ability to maintain appropriate professional boundaries and manage secondary traumatic stress and compassion fatigue.

Section 4: Working Conditions

  • Work is performed across office, field, and community-based settings depending on client and program needs.
  • May require regular travel to client homes, community sites, schools, courts, or partner agencies.
  • Standard business hours apply, with occasional evenings or weekends required as determined by program or client need.
  • Employee may be exposed to emotionally sensitive and high-stress situations involving trauma, crisis response, or work with vulnerable populations.

Section 5: Physical Requirements

  • Ability to sit, stand, walk, and travel to various locations as required by job duties.
  • Must be able to operate standard office equipment and computer systems with or without reasonable accommodation.
  • Valid driver's license and reliable personal transportation may be required depending on program assignment.

Pay: $40.00 - $90.00 per hour
