Social Worker

Heartland Living & Rehabilitation is currently hiring. Our facility provides residents with a full range of long-term care and rehabilitation services, along with a variety of engaging social and recreational activities. We currently have an opportunity for an energetic and dynamic Social Worker in our Greensboro, NC facility.

Qualifications

• Bachelors degree in Social Work or other human services area such as sociology, special education, rehabilitation counseling or psychology

• At least two years of experience in long term care
  Excellent verbal and written communication skills

• Experience in completion of MDS, CAA and CP, BIMS and PHQ9

• Strong organizational skills as evidenced by the ability to multi-task

• Have compassion for the elderly and for their families

• Proven ability to work in a team environment

• Demonstrated investigation skills related to customer complaints and other resident concerns

• Competency in electronic charting, PointClickCare preferred

• Strong working knowledge of Outlook, Microsoft Word, Microsoft Excel
