Overview
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will be responsible for providing comprehensive support and counseling services to individuals and families, particularly focusing on children and those with developmental disabilities. This role requires a strong understanding of mental health issues, crisis intervention, and case management. The Social Worker will work collaboratively with other healthcare professionals to ensure the best outcomes for our clients.

Duties

  • Conduct assessments to determine the needs of clients and develop appropriate treatment plans.
  • Utilize motivational interviewing techniques to empower clients in their recovery journey.
  • Maintain accurate medical documentation in compliance with HIPAA regulations, including intake forms, patient assessments, and treatment progress notes.
  • Collaborate with multidisciplinary teams to coordinate care for patients.
  • Provide home and community care services to enhance client well-being outside of traditional settings.

Experience

  • A Master's in Social Work or a related field is required; licensure prefferred .
  • Experience working with families in various settings such as hospitals or community organizations is highly desirable.
  • Familiarity with EMR systems for managing patient records effectively.
  • Experience working with diverse populations including geriatrics and individuals with autism or other disabilities is beneficial.
  • Hospice experience is a plus.

Job Type: Full-time

Pay: $60,269.98 - $70,000.00 per year
