Social Worker Case Manager for Adult Day Care Cente

VCare Holyoke Adult Day Health Program provides compassionate, culturally sensitive care and services to help our participants remain healthy, independent, and safe in the community, thus improving their quality of life while reducing their need for frequent hospitalizations.

Job Description:

We are seeking a dedicated and compassionate Case Manager/Social Worker to join our Adult Day Care Program. The successful candidate will be responsible for managing and coordinating services for our clients, ensuring they receive the best possible care and support tailored to their individual needs.

Key Responsibilities:

• Client Assessment and Intake:
• Conduct comprehensive assessments to determine the needs and eligibility of potential clients.
• Develop individualized care plans based on the assessment findings.
• Facilitate the intake process, including the collection of necessary documentation.
• Care Coordination:
• Coordinate and monitor the delivery of services and supports as outlined in the care plans.
• Maintain regular communication with clients, families, caregivers, and other service providers.
• Adjust care plans as needed based on ongoing assessment and feedback.
• Advocacy and Support:
• Advocate for the rights and needs of clients within the Adult Day Care Program and the broader community.
• Provide emotional support and counseling to clients and their families.
• Assist clients in accessing community resources and services.
• Documentation and Reporting:
• Maintain accurate and up-to-date client records, including case notes, assessments, and care plans.
• Prepare and submit required reports and documentation in a timely manner.
• Ensure compliance with all relevant regulations, policies, and procedures.

Job Type: Temporary

Pay: $20.00 - $25.00 per hour
