The Social Worker – Disability Waiver Case Manager responsibilities include providing Case Management Services and related conditions, assisting individuals in gaining access to needed social, medical, educational and other supports and services.
Assist persons on a waiver to access needed waiver and State plan services, as well as needed medical, social, educational and other services, regardless of the funding source. Assure service planning and delivery are done through a person-centered approach. Assist in monitoring the delivery of services and provider compliance with quality assurance standards. Complete other functions to support agency operations.

Minimum Qualifications
Education and Experience
- A bachelor's degree from an accredited four-year college or university with a major in social work, psychology, sociology or closely related field or
- A bachelor's degree from an accredited four-year college or university with a major in any field and one year of experience as a social worker in a public or private social services agency or
- A public health nurse or
- A registered nurse
Required Training During Orientation
- Training on Person Centered Thinking and Planning
- Mandated Reporter Training
- Annual Data Privacy, Security, Fraud and OSHA Training
- SSIS and MMIS Training
- Other trainings as related to position
Required Certifications/Licenses
- Valid driver’s license and ability to maintain such license and proof of insurance.
Preferred Certifications/Specialties
- Social Worker Licensure preferred
Relationships
- Individuals needing long term services and support and their families and/or legal guardians.
- Regular interaction with other County Social Workers, Public Health and Attorney office staff.
- Tier 2 and Tier 3 service providers, medical, psychiatric, psychological, legal, educational, vocational, rehabilitation and other professionals and resources.
- Department of Human Services staff.
Supervisory Responsibilities
No direction supervision of employees.
Working Conditions
- Work is mainly performed in an office setting but may also be in clients’ homes, residential facilities, and other county offices.
- Frequent travel may involve exposure to inclement weather conditions, unsanitary conditions in homes and unsafe environments.
- Direct service provision may require dealing with persons who are disoriented, hostile, aggressive, abusive or threatening.
- There may be hours worked outside of the normal business hours.
