A vacancy exists in the Department of Community Health Services, Assessments Evaluation Review Services.

Definition of Responsibility
The Social Worker III will be responsible for collaborating with other human service agencies, supervising clinical staff, monitoring budgets and grant expenditures, working with providers, monitoring contracts for performance and compliance, preparing grant proposals and reporting on service/program/initiative implementation, which includes a multi-tiered system of supports.

Job Purpose
Under general supervision, Social Worker III in Assessments, Evaluation and Review Services (AERS) program provides advanced clinical and administrative leadership. This role plays a central part in ensuring high-quality service delivery across the Preadmission Screening and Resident Review (PASRR) program and AERS Case Management functions.

Examples of Essential Job Duties

  • Supervises licensed professional and human services support staff engaged in health and social services aligned with Health Department standards
  • Selects, trains, supervises and evaluates social workers and other professional and support personnel.
  • Provides or coordinates training related to behavioral health and developmental disorders across the lifespan.
  • Conducts and supervises complex psychosocial assessments for individuals with medical, psychiatric, neurocognitive or developmental needs.
  • Familiarity with the major diagnostic categories of the DSM-5TR, ability to recognize patterns of symptoms, diagnostic criteria and knowledge of treatment modalities across the lifespan.
  • Reviews and approves clinical documentation, care plans and recommendations.
  • Provides consultation to internal staff, partner agencies, hospitals and long-term facilities.
  • Day to day oversight of AERS program case assignments in conjunction with nursing leadership.
  • Provides day to day oversight of PASRR screening and Level II processes ensuring compliance with federal and state regulations.
  • Leads and coordinates regular PASRR[ and Case Management Case Conferences to strengthen interdisciplinary expertise.
  • Leads and supports AERS Case Management operations including work flow development, quality assurance and staff guidance.
  • Coordinates the program's activities with other agencies, Departments of Social Services and Aging specifically, and programs which provide services to families and individuals.
  • Develops and implements program policies, procedures and performance improvement initiatives.
  • Monitors PASRR program data, prepares reports for outcomes and performance and supports compliance reviews.
  • Participates in the review of the budget as it relates to PASRR billing.

  • Builds and maintains strong working relationships with hospitals, nursing facilities, behavioral health partners and community organizations.
  • In a leadership capacity, represents AERS in interagency meetings, workgroups and stakeholder discussions.
  • Offers training and technical assistance related to advanced clinical social work practice related to behavioral and developmental disorders to staff and external partners on PASRR process and case management best practices.

Qualifications

Required Education and Experience
Possession of a master's degree in social work from a college or university approved by the Council of Social Work Education.
Plus
Four years' experience rendering professional social work services.

Education beyond a master's degree from an accredited college or university in the field of social work may be substituted on a year-for year basis, up to a maximum of two years for the required experience.

Preferred Experience

  • Experience supervising licensed social work and non-licensed human services staff.
  • Knowledge of social work practice and clinical standards and guidelines.
  • Professional social work experience, including experience with complex case management, behavioral health, developmental disabilities and long-term care systems.

Salary

Annual Salary review - Step increases may be granted on anniversary date based on satisfactory job performance
