Join the Case Management and Social Work team at Jamaica Hospital Medical Center, a 384-bed, award-winning teaching hospital and Level I Trauma & Stroke Center. As a Medical Social Worker, you’ll provide critical support to patients and families, coordinate safe discharges, and help patients navigate complex medical and social needs.

Position Highlights

• Full-Time
• Salary: $43.90 – $58.02 per hour (based on experience)

• Rotating weekends and holidays required

Responsibilities

• Conduct biopsychosocial assessments and discharge planning.
• Provide counseling and crisis intervention for patients and families.
• Connect patients with community, financial, and social service resources.
• Collaborate with interdisciplinary healthcare teams.
• Advocate for patients’ psychosocial and emotional needs.
• Facilitate referrals and support services as needed.
• Investigate and address cases involving suspected abuse or neglect.

Requirements

• Master’s Degree in Social Work (MSW).
• NYS LMSW or LCSW required.
• Hospital experience preferred; new graduates welcome.
• Strong communication, counseling, and organizational skills.
• Knowledge of community resources and discharge planning.
