About Ashley House:
Ashley House is a leader in pediatric care, dedicated to enriching lives and fostering wellness. Our 15-bed facility, in partnership with Seattle Children’s Hospital (SCH), provides a supportive environment where children can thrive. We focus on normalizing experiences and offer educational and recreational opportunities, as well as engagement outside the medical setting. Our innovative program equips families with the training and tools needed to smoothly transition children with complex medical conditions from hospital to home.
The Opportunity
We are seeking an experienced Social Worker with exceptional problem-solving skills to join our brand-new Pediatric Skilled Nursing Facility. As a key member of our social work team, you will deliver case management services for medically fragile children enrolled in Ashley House’s Bridges to Home Program. This role involves close collaboration with clients, families, state agencies, physicians, hospitals, schools, and facility staff. You will provide post-placement care and ongoing support to families, including regular follow-ups and in-home visits for up to six months to ensure successful adjustment and stability. Ashley House is a growing agency, offering opportunities to help develop new programs serving the medically fragile community.
Qualifications
To be considered for this opportunity, the following are required but not limited to:
- A Bachelor’s degree in Social Work or related field, MSW preferred. Minimum of 2 years of experience as a Social Worker OR at least 2–3 years of relevant experience in a similar role. Relevant experience may include working with medically complex individuals, vulnerable populations, or in healthcare, community health, or social services settings. Candidates must demonstrate the ability to assess client needs, develop care plans, coordinate services, and advocate effectively for clients and families
- Annual acceptable driving record, insurance, and a valid driver’s license.
- Ability to pass and maintain an acceptable criminal background check with Ashley House and the State of Washington, including personal references, and an FBI fingerprint criminal history check
- Proof of a valid Tdap, Flu, and COVID Immunization, as well as annual TB testing.
- Proof of a valid Washington Food Handler’s Permit from the Health Department
- Valid Healthcare Provider (BLS) CPR certification
- Ability to participate in rotating on-call responsibilities. When on call, you must be available 24 hours a day, 7 days a week during assigned on-call periods. Occasionally, you must be available to respond in person outside of normal business hours, especially in crisis situations

Pay: $81,860.89 - $100,435.18 per year
