Key Responsibilities:
- IV Therapy Administration:
- Initiate and manage IV therapy including antibiotics, hydration, pain management, and total parenteral nutrition (TPN).
- Perform venipuncture and manage peripheral or central venous access devices (e.g., PICC lines, ports).
- Patient Assessment:
- Conduct comprehensive nursing assessments prior to and during IV therapy.
- Monitor for complications such as infiltration, phlebitis, allergic reactions, or infections.
- Educate patients and caregivers on IV therapy, medication side effects, and catheter care.
- Care Coordination:
- Collaborate with physicians, pharmacists, and the home health team to develop and update individualized care plans.
- Communicate patient status and therapy effectiveness to appropriate providers.
- Documentation:
- Accurately document assessments, interventions, patient responses, and progress notes in EMR or paper charting system as required.
- Maintain up-to-date records in compliance with agency and regulatory standards.
- Compliance and Safety:
- Follow infection control protocols and aseptic techniques.
- Ensure proper storage, handling, and disposal of IV medications and supplies.
- Adhere to all local, state, and federal regulations, including HIPAA
