The Role

As our SOC Nurse, you'll complete initial home evaluations for pediatric patients, establish care plans, and set the tone for a safe, high-quality home health experience. You'll be a key touchpoint for families at the most important stage of their care journey.

What You'll Do

• Conduct comprehensive start of care assessments in the home setting
• Develop and initiate individualized care plans based on physician orders
• Educate family caregivers on care needs, equipment (vents, g-tubes, pumps), medications, and safety
• Complete thorough, accurate clinical documentation
• Communicate findings and care plans to the clinical team and coordinating nurseCore

Requirements

Please only apply if you meet all of the following:

• Active Florida RN license + BLS certification
• 1 year + experience completing Start of Care visits in home health
• Strong clinical documentation and EMR experience
• Valid Drive License & Reliable transportation and willingness to travel to patient homes
• Comfort working with medically complex pediatric patients

Preferences

• Bilingual English / Spanish (especially for Miami-Dade and Broward)
• Experience with Statewise EMR
• Pediatric home health or private duty nursing experience
