What You'll Do:
Under the direction of the Director of Clinical Services, the Supervisor, Case Manager oversees the work and supervision of assigned Social Services staff while maintaining a direct care caseload. This role combines leadership and clinical responsibilities to support high-quality patient care and efficient unit operations.
Supervisor responsibilities represent approximately 20–25% of the role and include staff onboarding, training, scheduling, timecard oversight, and performance management. Direct care responsibilities represent approximately 75–80% of the role and include performing standard Case Management duties within the Observation Unit.
The Supervisor, Case Manager collaborates closely with the interdisciplinary treatment team to coordinate care and discharge planning for patients and their families. This role also partners with internal staff, community providers, and referral sources to ensure continuity of care. The Supervisor supports admissions, discharge planning, collateral gathering, and legal processes while ensuring the unit operates safely and efficiently.
- Staff Supervision and Management:
- Direct, supervise, and evaluate assigned Case Management staff under the guidance of supervisor and clinical leadership.
- Provide oversight of department staff on assigned shifts.
- Participate in the interview and selection of Case Management staff.
- Responsible for onboarding and training new hires, including completion of 90-day Competency Checklists.
- Approve timecards, PTO, manage performance, and handle disciplinary actions for assigned staff.
- Staffing and Scheduling:
- Maintain adequate staffing to ensure proper staff-to-patient ratios.
- Adjust staffing based on census, PTO, and business needs.
- Limit overtime through use of Pool staff.
- Caseload Management:
- Manage assigned caseload on Observation Unit, facilitating admission triages and implementing individualized, person-centered Treatment Plans.
- Complete signed Treatment Plans and review with patients and involved parties.
- Gather collateral information to assess immediate treatment and discharge needs.
- Monitor available community beds and observation census.
- Care Coordination and Collaboration:
- Actively coordinate care to ensure services are integrated with facility staff.
- Identify gaps in service delivery and make recommendations to meet patient needs upon discharge.
- Collaborate with interdisciplinary treatment team to engage, monitor, and communicate with patients while maintaining safety.
- Documentation and Recordkeeping:
- Maintain clinic records, documenting all services and patient activities, including brief interventions, care coordination, discharge planning, and treatment planning.
- Obtain patient signatures as necessary.
- Legal and Regulatory Compliance:
- Maintain knowledge of the Title 36 Involuntary Commitment process and monitor petition timeliness and completeness.
- Attend COE hearings when subpoenaed.
- Understand applicable County RBHA systems and potential funding sources.
- Professional Development and Supervision Participation:
- Participate in individual and group supervision sessions.
- Engage in ongoing education, including in-services and training, to maintain and improve competency.
- Demonstrate competency through post-testing, skill observation, and supervisor assessment.
- Clinical Oversight (if licensed):
- Sign off on Psychosocial assessments for non-licensed staff in assigned programs within required timeframes.
- Additional Duties:
- Perform all other duties as assigned.
What You'll Bring:
- Associate's degree in behavioral health or related field.
- 3-5 years of psychiatric, behavioral health, or relevant experience.
- 1-2 years of managerial/supervisory experience in a healthcare setting.
- Fingerprint clearance required (for facilities that have youth services).
- Knowledge of and experience with the AZ Title-36 legal process (AZ only).
- Knowledge of and experience with the COT legal process for state of practice.
- Arizona Level 1 Fingerprint Clearance Card.
- The Company has a mandatory vaccination policy. All successful applicants must be fully vaccinated, including showing proper documentation, or otherwise be exempt pursuant to the Company's exemption process prior to their start date as a condition of employment
It would be great if you had:
Licensed in the state of practice or the ability to obtain licensure.
Bachelor's degree in behavioral health or related field with at least 4 years of related experience.
Master's degree in behavioral health field with at least 2 years of related experience.
Experience in an inpatient or crisis healthcare setting.
