Pay: $45,000.00 per year/caseload of 30 individuals

Job description:

Intensive case management supporting adults with I/DD who receive services under DDD. Ensuring, monitoring and documenting that these individuals are provided high-quality, person-centered services and providing supports so that they have access to necessary resources and can live independently in their communities.

Requirements:

• Bachelor’s Degree in a Human Services or related field
• 2 years experience working with adults with I/DD
• Valid Driver’s License and reliable transportation
• Standard work hours: Monday to Friday, 9:00 AM – 4:30 PM
