Job Summary
We are seeking a dedicated and compassionate Targeted Case Manager to join our dynamic healthcare team. In this vital role, you will coordinate comprehensive care plans for patients with complex medical and behavioral health needs, ensuring seamless access to services across inpatient, outpatient, and post-acute settings. Your expertise will help improve patient outcomes by providing personalized support, facilitating discharge planning, and managing utilization to optimize care delivery. Responsibilities

  • Certified Behavioral Health Case Manager (CBHCM)
  • Conduct thorough patient assessments, including medical, behavioral health, and social determinants of health factors, utilizing your knowledge of physiology and medical terminology.
  • Develop individualized care plans that address acute care needs, behavioral health challenges, addiction counseling, and post-acute care requirements.
  • Coordinate with hospital teams—including ICU, Emergency Medicine, Level I and Level II Trauma Centers, PICU (Pediatric Intensive Care Unit), and surgical units—to facilitate smooth patient transitions from inpatient to outpatient or community-based services.
  • Manage utilization review and management processes by evaluating diagnostic evaluations, medical records, and discharge planning documentation to ensure appropriate level of care.
  • Collaborate with social workers, mental health counselors, and healthcare providers to support patient service goals and promote holistic recovery strategies.
  • Facilitate intake procedures for new patients and oversee ongoing case management activities across inpatient and outpatient settings.
  • Maintain HIPAA compliance by accurately documenting patient information in medical records while respecting confidentiality standards.
  • Engage in discharge planning activities that include coordination with managed care organizations and post-acute providers to ensure continuity of care.
  • Support diagnostic evaluations and behavioral health interventions tailored to pediatric or adult populations as needed.

Requirements

  • Bilingual: Spanish & English required.
  • Proven experience working within hospital environments such as Level I or Level II trauma centers, including ICU or emergency medicine settings.
  • Strong background in acute care, inpatient management, or outpatient clinic experience with a focus on hospital medicine or behavioral health.
  • Knowledge of managed care processes, utilization management, patient assessment techniques, and discharge planning protocols.
  • Familiarity with medical terminology, diagnostic evaluation procedures, and the use of electronic medical records systems.
  • Experience working with diverse patient populations including pediatrics (PICU), mental health clients, or individuals requiring addiction counseling.
  • Excellent communication skills combined with the ability to collaborate effectively across multidisciplinary teams.
  • Ability to handle sensitive information with discretion in accordance with HIPAA regulations.
  • Demonstrated understanding of physiology principles relevant to acute care scenarios and post-acute recovery pathways. Pay: $26.00 per hour
