Position Summary:

Bridges Community Support Services is currently seeking a Targeted Case Manager (TCM). The TCM will provide mental health case management services to clients with severe and acute diagnosis as recommended by Medicaid. The TCM will utilize experience and knowledge to assess needs of the clients and their families in order to link and coordinate the appropriate resources. They are responsible for managing an assigned caseload responding appropriately to client needs in compliance with Medicaid documentation standards. This includes assessing client needs, developing, implementing and reviewing service plans, and working with other community resources in achieving client service needs.

This position is for high intensity clients who received both Targeted Case Management and Housing with tenancy sustaining services. These clients may be homeless or have tentative housing conditions. The TCM will be required to provide quality services that prevent hospitalization and support stability with qualitative results.

Case Manager Responsibilities:

• Assessment of client with ongoing updates as needed
• Complete Individual Service Plan in accordance with assessment of client's needs
• Provide services as determined by ISP goals (Link, coordinate, monitor, evaluate, review progress and advocate for services)
• Complete daily documentation following service plan with regards to Medicaid timelines and guidelines
• Complete Intakes, timely client reviews and discharges for assigned case load
• Maintain case load compliance spreadsheet with up to date forms and dates obtaining appropriate signatures.
• Attend staff meetings, client care, supervision and trainings
• Maintain TCM certification, CEU's and monthly in-service trainings

This position requires home visits for intakes as your caseload allows and monthly face to face visit with your client.

Qualifications and Education Requirements

• Bachelor’s Degree in psychology, social work, counseling, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field.
• A minimum of 1 year or a full-time or equivalent working with adults with serious emotional/mental health disturbances.
• The Florida Certification Board: Case Management Certification (Preferred)
• Must be able to communicate verbally and written in a prompt, professional and positive manner.
• Intermediate knowledge and skills using Microsoft Office.

Other Requirements :

• Be required to possess a valid driver's license for the operation of a vehicle as it is necessary to perform the essential duties of the position.
• Eligible for Florida Certification

Pay: $21.60 - $25.96 per hour
