Third Shift Bilingual (Spanish) Nurse Care Provider - Remote Patient Care

Job Overview
We are seeking a dedicated and compassionate Bilingual Care Provider to join our healthcare team. The ideal candidate will play a crucial role in coordinating remote patient care, ensuring that individuals receive the appropriate medical services and support they need. This is a customer service focused position and requires a strong understanding of patient needs, excellent communication skills, and the ability to work collaboratively with healthcare professionals to enhance patient outcomes.

Duties

  • Provide customer service including scheduling of patient appointments and translating as needed.
  • Monitor patient progress and adjust care plans as necessary to ensure optimal health outcomes.
  • Maintain accurate medical records and documentation in compliance with HIPAA regulations.
  • Coordinate with various departments to facilitate timely access to medical services, including primary care, inpatient, and critical care settings.
  • Educate patients and their families about health management, treatment options, and available resources.
  • Conduct regular follow-ups to ensure continuity of care and address any emerging health concerns.
  • Advocate for patients’ needs within the healthcare system.

Qualifications

  • Current nursing license (RN or LPN) is required; Compact license preferred, or ability to obtain.
  • Fluent in both English and Spanish
  • Familiarity with primary care practices and inpatient care protocols.
  • Strong knowledge of HIPAA regulations to ensure patient confidentiality and data protection.
  • Excellent interpersonal skills with the ability to communicate effectively with patients, families, and healthcare teams.
  • Ability to work independently as well as part of a collaborative team environment.
  • Previous experience in remote patient monitoring and remote chronic condition management preferred.

Job Type: Full-time

Pay: $67,937.00 - $73,561.00 per year
