Time Limited Subsidy Housing - Case Manager

We provide emergency and permanent housing options and other community-based services that create opportunities for them to lead productive lives. We operate in Los Angeles and community East and South East of Los Angeles County.

JOB TITLE: Time Limited Subsidy Housing Case Manager

The primary responsibility of TLS Housing Case Manager is to provide assessment, case management, crisis intervention, housing identification, and coordinate activities, including management of rental subsidies for transitional age youth that are homeless. The Time Limited Housing Case Manager will help youth develop the appropriate connections and steps in life to become more self-sufficient and independent by accessing permanent housing and or while pursuing higher education. From time to time, program director or executive staff may ask to conduct additional activities or tasks necessary to the operations of the organization and or the project.

DUTIES AND RESPONSIBILITIES

The Time Limited Housing Case Manager will work as part of a team under the direct supervision of the Permanent Housing Program Director. Under the supervision of the Permanent Supportive Housing Program Director and assigned Supervisor the position will:

Direct Services:

  • Coordinate outreach strategy and referral processes for youth.
  • Assess youth for skills, challenges and appropriate services.
  • Elaborate with eligible clients Individualized Service Plan that will be updated monthly.
  • Provide case management services to clients, including developing a rental subsidy disbursement plan and identifying the right options for permanent supportive housing.
  • Provide clients with referrals as needed for onsite and offsite services and activities, in particular to mental health and substance abuse services and to opportunities for educational & career development and personal enrichment.
  • Evaluate transportation needs and support them in developing a sustainable plan.
  • Provide clear direction, support and follow up to clients.
  • Support clients in action plan to resolve issues that can prevent clients from maintaining their housing.
  • Provide support to tenants as needed to deal with any issues related to housing retention.
  • Create and maintain a directory of service providers and resources. Cultivate partnerships with community stakeholders and public agencies to expand opportunities for youth in the program.
  • Expand the list of resources and providers to assure there are no gaps in services

Administrative

· Achieve positive program and contract outcomes,

· Ensure compliance with data managing system,

· Maintain clients’ file according to agency and contract policies,

· Prepare and submit programmatic reports, client notes, and all other documentation in a timely manner,

· Maintain files in a secure and locked cabinet,

· Follow at all times the agency code of conduct,

Other duties and responsibility may be developed by immediate supervisor according to program and agency needs.

SITE AND SUPERVISION

The Time Limited Housing Case manager will work as part of a team under the direct supervision of the Permanent Supportive Housing Program Director and supervisor. From time-to-time case manager may be asked by the program manager or executive staff to conduct activities or tasks necessary to the operations of the organization.

Services will be provided at youth residences, in the community, on community college campuses, and at Jovenes, Inc. offices, located in Boyle Heights.

SKILLS AND ABILITIES REQUIRED TO PERFORM THE ESSENTIAL JOB FUNCTIONS

Communication Skills and Abilities:

  • Ability to effectively present information and respond to questions from clients, office personnel, other employees and the general public
  • Ability to read, analyze and interpret manuals and office documents

Mental/Interpersonal Skills and Abilities:

  • Ability to define problems, gather, transcribe and post data, analyze, synthesize and classify information, and draw valid conclusions
  • Ability to work independently and with others – including other employees, clients and members of the public -- in face-to-face and telephonic contexts
  • Ability to multi-task, establish priorities and manage time, meet deadlines and perform duties under time constraints

Physical Skills and Abilities:

  • Ability to talk or hear in order to give and receive information and instructions
  • Ability to stand, walk, sit, use hands to finger, handle or feel objects, tools or controls
  • Ability to reach with hands and arms
  • Ability to use computer keyboard up to 50% of the day
  • Visual acuity, ability to adjust focus and peripheral vision for work on a laptop or other computer equipment
  • Ability to lift and/or move up to 20 pounds

Equipment and Computer Software Skills and Abilities:

· Computer literate (basic working knowledge of Microsoft WORD, Excel, and Outlook)

· Able to use telephone, typewriter, office copier, calculator, fax machine, computer printer, and scanner, or other equipment as required

EDUCATION, EXPERIENCE, CERTIFICATES & TRAINING

  • Bachelor’s degree in social work, counseling or similar or 4 years of experience in a relevant mental health setting;
  • Experience working with transitional age youth, individual with mental disabilities, low-income families.
  • Demonstrated ability to work with a culturally diverse clientele & staff
  • Valid California driver’s license and personal vehicle insurance.

This is a full-time position. Experience will be taken into consideration in regard of salary rate. Salary range: $49,920-$56,140.00. Health, dental and life insurance provided. Eligible for paid vacations, sick time, holidays, agency’s 401K plan and matching contribution according to agency policies. Must have your own car. Must be willing to use your car for work related travel and client’s transportation as needed. Mileage reimbursement provided.
