Organizes and schedules necessary appointments, tests, and consultations with various healthcare providers, including transplant teams, surgeons, and cardiologists. Assesses patient eligibility for advanced therapy or organ transplantation by reviewing medical records, conducting preliminary tests, and consulting with specialists. Coordinates initial referral processing and patient evaluations for advanced therapy, potential organ recipients, living donors, or VAD patients. Provides detailed education to patients and families about the treatment process, potential risks, benefits, and post-treatment care. Maintains accurate and up-to-date records of all patient interactions, assessments, and care plans. Acts as a liaison between the patient, their family, and the healthcare team, ensuring effective communication and addressing any concerns. Monitors patient progress post-treatment or post-transplantation, schedules follow-up appointments, manages complications, and provides ongoing support. Ensures patients adhere to prescribed medications and treatments, educating them about compliance and managing side effects. Collaborates with healthcare providers to develop and implement comprehensive care plans. Offers emotional and psychological support to patients and families, addressing concerns and providing mental health resources. Participates in quality improvement initiatives to enhance patient care and outcomes. Stays informed about advancements in transplant medicine and incorporates evidence-based practices into patient care. Other duties as assigned.Knowledge, Skills, and Abilities:Pay Range:
