Job Description:

Organizes and schedules necessary appointments, tests, and consultations with various healthcare providers, including transplant teams, surgeons, and cardiologists. Assesses patient eligibility for advanced therapy or organ transplantation by reviewing medical records, conducting preliminary tests, and consulting with specialists. Coordinates initial referral processing and patient evaluations for advanced therapy, potential organ recipients, living donors, or VAD patients. Provides detailed education to patients and families about the treatment process, potential risks, benefits, and post-treatment care. Maintains accurate and up-to-date records of all patient interactions, assessments, and care plans. Acts as a liaison between the patient, their family, and the healthcare team, ensuring effective communication and addressing any concerns. Monitors patient progress post-treatment or post-transplantation, schedules follow-up appointments, manages complications, and provides ongoing support. Ensures patients adhere to prescribed medications and treatments, educating them about compliance and managing side effects. Collaborates with healthcare providers to develop and implement comprehensive care plans. Offers emotional and psychological support to patients and families, addressing concerns and providing mental health resources. Participates in quality improvement initiatives to enhance patient care and outcomes. Stays informed about advancements in transplant medicine and incorporates evidence-based practices into patient care. Other duties as assigned.Knowledge, Skills, and Abilities:
  • Ability to communicate verbally and in written form in English with physicians, patients and staff of varied backgrounds in a respectful, effective and professional manner [Required]
  • Strong interpersonal, organization skills and the ability to work effectively in an environment with frequent interruptions [Required]
  • Ability to act in an autonomous, self-directed manner while maintaining the ability to collaborate with professionals and non-professionals in situations of high complexity and high intensity [Required]
  • Self-motivated with the ability to multi-task and adapt to changing demands [Required]
  • Mature judgment and the ability to handle confidential information within Florida Hospital guidelines and application regulations [Required]
  • Knowledge of clinical documentation, exam procedures and protocols [Required]
  • Strong communication and organizational skills [Required]
  • Basic clerical skills and the ability to type 20 WPM [Required]
  • Bilingual (English required/Spanish Preferred) [Preferred]
  • Medical terminology [Preferred]

Education:
  • Associate [Preferred]
  • Bachelor's [Preferred]

Field of Study:
  • within the business and/or health care fields

Work Experience:
  • 1+ experience in a complex practice environment or healthcare setting such as catheter lab, endoscopy, infusion center, etc., coordinating multiple physician office, with direct care or working in an inpatient care area within the last three years [Required]
  • Experience in critical care, dialysis, hepatology, cardiology, pulmonology or transplant clinical setting [Preferred]

Pay Range:

$18.14 - $29.02
