GENERAL SUMMARY

The Transportation Coordinator will assist with entering timely transportation services within our EMR and Bambi system. Transportation Coordinator will be responsible for entering up to 25-30 rides a day for patients going to and from medical appointments at Cooper Green Mercy Health Authority and referred external services. Coordinator will also be responsible for communicating with patients, confirming demographic and contact information to ensure accuracy of transportation pick up and drops. While coordinating with contracted vendors dispatch team, Cooper Green Mercy Health social services department, staff, and providers. Coordinator will be responsible for maintaining accuracy and compliance with the transportation program and policy. Communicating any changes or barriers with our transportation program to Department of Social Services Manager.

KEY RESPONSIBILITIES
  • Transportation Coordinator will be responsible for entering up to 25-30 rides a day for patients going to and from medical appointments at Cooper Green Mercy Health Authority and referred external services.
  • Coordination with contracted vendors dispatch team, Cooper Green Mercy Health social services department, staff and providers.
  • Transportation Coordinator will be responsible for maintaining accuracy and compliance with the transportation program and policy.
  • Collaborate with licensed Social Workers within the clinics of Cooper Green Mercy Health Services Authority to ensure patients’ social services needs are addressed.
  • Communicating any changes or barriers with our transportation program to Department of Social Services Manager.
  • Other duties as assigned.
Position Requirements:
MINIMUM QUALIFICATIONS

Required: High School Diploma or equivalent, with a minimum of one (1) year of experience in social services, case management, or a similar role within a healthcare setting or related field.




WORK ENVIRONMENT

  • The on-site, office based role is full time, with the following options as work hours:
o 7:00 am - 3:30 pm CST
o 7:30 am – 4:00 pm CST
o 8:30 am – 5:00 pm CST
  • This role may require lifting up to 50 lbs.
