TITLE: Transportation Driver
REPORTS TO: Administrator/ Transportation Supervisor
METHOD OF PAY: Hourly
PURPOSE OF POSITION:
The primary purpose of your job position is to provide transportation in a safe and timely manner, in accordance with established policies and procedures, and as directed by your supervisor.
ROLE and RESPONSIBILITIES:

Communicate with dispatcher on Two Way Radio

Be able to operate a Hydraulic Lift.

Report to Dispatcher and pick up patient schedule daily.

Transport 5 - 6 patients per day to and from doctors’ appointments.

Inspect van daily. (ex: check oil, tires, first aid kit & fire extinguisher)

Ensure that all passengers are secured by seat belts.

Complete all necessary forms, (ex: DMA 408, van log, etc.)

Collect co-payments and private payments from patients
-
Other related duties and responsibilities that may become necessary to assure that the needs of the facility are met.

Highly focused and strong attention to detail

Ability to follow written and oral instructions

Ability to deal tactfully with personnel, residents, visitors and the general public.
EDUCATION/EXPERIENCE/QUALIFICATIONS:

Be a minimum of 25 years of age and of good moral character.

Must have CPR and First Aid License.

Must have completed the Police Traffic Course.
