Requirements

Home Health experience – Required

Active California PT License – Required

BLS/CPR Certification – Required

Valid Driver’s License – Required

Must be 21 years of age or older

Master’s Degree – Preferred

Essential Functions

Complete comprehensive patient evaluations

Develop and implement individualized treatment plans

Address assessment areas requiring clinical attention

Maintain accurate and timely documentation

Ensure compliance with state, federal, and agency guidelines

Collaborate with interdisciplinary teams to support patient outcomes
