Are you passionate about supporting individuals on their journey to recovery and independence? Join our dedicated team as a Case Manager I in the Treatment Apartment Program (TAP), an OMH-licensed scattered site housing initiative. This role is essential in providing person-centered case management and support services to individuals with Serious and Persistent Mental Illness (SPMI), including those with co-occurring disorders and housing instability.
You’ll help participants build life skills, access housing and behavioral health services, and foster community and family connections—all while promoting wellness and stability.

This position typically follows a Monday–Friday, 8:00 AM to 4:30 PM schedule, with flexibility as needed to meet program and participant needs. Certain deadlines and unanticipated developments may require work during evenings, weekends.
