Position Overview
We are seeking a highly organized, empathetic, and detail-oriented Alumni Coordinator to oversee post-treatment engagement for a large alumni population. This role emphasizes regular (preferably monthly) outreach, facilitating monthly alumni meetings, managing high-volume client tracking, and producing high-quality clinical documentation to support relapse prevention, outcome measurement, and continuity of care.
Key Responsibilities
- Manage and track a large caseload of alumni (hundreds to thousands cumulatively) through systematic, proactive follow-up to monitor long-term recovery progress.
- Perform regular outreach—preferably monthly—via phone, text, email, and other channels to assess well-being, provide encouragement, identify potential relapse risks, offer resources, and document interactions.
- Plan, organize, and facilitate monthly alumni meetings (virtual and/or in-person), including scheduling, promotion, content development (recovery-focused topics), facilitation, attendance tracking, and post-meeting follow-up.
- Maintain excellent, detailed clinical documentation in the organization's EMR/CRM system, including progress notes, recovery status updates, relapse indicators, engagement metrics, referrals made, outcome data, and any clinical observations from alumni interactions—ensuring all records are accurate, timely, objective, and fully compliant with HIPAA and 42 CFR Part 2 regulations.
- Document alumni milestones, feedback, success stories, and barriers to recovery (with proper consent) to inform program improvements, risk assessment, and potential re-engagement strategies.
- Coordinate clinical aftercare referrals (e.g., outpatient therapy, sober living, peer support groups, or readmission) and collaborate with clinical/case management teams to ensure seamless support transitions.
- Develop additional engagement initiatives, such as sober events, workshops, mentorship programs, and holiday gatherings to strengthen the alumni community.
- Track and report key performance metrics (e.g., monthly outreach completion rates, meeting attendance, engagement trends, relapse prevention indicators, and alumni satisfaction) through detailed summaries and data dashboards for leadership review.
- Serve as the primary, supportive point of contact for alumni, fostering motivation and connection to ongoing recovery resources.
AHCCCS Targeted Investments (TIP) Program Responsibilities
- Actively support the organization's participation in Arizona's AHCCCS Targeted Investments Program (TIP).
- Collaborate closely with the internal TIP team (including quality improvement, compliance, and data staff) to ensure alumni follow-up activities align with TIP milestones, such as tracking recovery outcomes, social determinants of health (SDOH) screening/referrals, and population health metrics.
- Collect, compile, and submit required alumni/alumni-engagement data (e.g., follow-up contacts, engagement rates, relapse prevention indicators, referral outcomes, and progress notes) for TIP reporting purposes, ensuring accuracy and timeliness in line with program requirements.
- Attend regular TIP-related meetings, including internal team huddles, Quality Improvement Collaborative (QIC) sessions (facilitated by partners like Arizona State University), and any AHCCCS-mandated webinars or check-ins to stay current on milestones, performance measures, and best practices for integrated aftercare.
- Contribute to TIP deliverables by documenting how alumni support efforts promote whole-person care, reduce fragmentation, and improve long-term health outcomes for AHCCCS-enrolled alumni.
Qualifications
- 1+ years of experience in addiction recovery support, alumni relations, case management, or high-volume client follow-up in a treatment setting.
- Lived experience in recovery highly valued (minimum 2 years continuous sobriety if applicable).
- Proven track record of managing large caseloads with consistent, timely follow-up and no dropped contacts.
- Strong skills in regular, frequent outreach (monthly preferred) and building rapport in sensitive recovery conversations.
- Exceptional attention to detail and proficiency in clinical documentation—including writing clear, concise progress notes, tracking clinical indicators, and maintaining compliant records in EMR/CRM systems (e.g., Salesforce, SimplePractice, or similar).
- Experience facilitating group meetings/events on a recurring (monthly) basis.
- Knowledge of recovery resources, 12-step/SMART Recovery programs, sober living, and aftercare planning.
- Excellent communication, empathy, and professional demeanor when handling confidential, clinical information.
- Proficiency with Microsoft Office/Google Workspace, CRM/EMR tools, and basic data reporting.
- Ability to work independently in a fast-paced environment and handle occasional evenings/weekends for meetings/events.
- Valid driver's license and reliable transportation preferred (for in-person activities in the Phoenix area).
