Overview
We are seeking a dedicated and compassionate LPN/RN to join our healthcare team. The ideal candidate will play a vital role in supporting pediatric patients in home care setting. This position is essential for ensuring that our patients receive the highest quality of care while assisting the parents in various tasks related to the child's care.

Responsibilities

• Provide direct patient care, including trach, vent, and G-tube management during assigned shifts.
• Conduct thorough patient assessments and monitor vital signs to ensure stability.
• Administer medications and perform catheterization as needed, adhering to HIPAA regulations.
• Document patient information accurately in EHR systems and communicate effectively with healthcare teams.
• Support patients with tube feeding and assist with daily living activities, ensuring comfort and safety.

Qualifications

• Valid RN or LPN license required
• Proficient in patient care and assessment
• Experience with tracheostomy, ventilators, and G-tubes
• Knowledge of catheterization and tube feeding
• Basic Life Support (BLS) certification
• Familiarity with EHR systems and HIPAA regulations
• Critical care and long-term care experience preferred
• Strong communication skills and ability to work with diverse populations
