We are seeking a licensed US Registered Nurse (USRN) to join our remote clinical care team, supporting Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. This role combines clinical review, lab follow ups, and patient communication, ensuring continuity of care for patients with chronic conditions.
The ideal candidate has strong clinical judgment, experience working within CCM and RPM workflows, and is comfortable using EMR systems, particularly eClinicalWorks (ECW). This is a full-time, hourly, non-voice focused role, with structured training followed by flexible scheduling.
Fully Remote Role: Work from anywhere with a stable internet connection
Flexible Scheduling: Training will follow US Eastern Time, with flexibility afterward across US and PH time zones
Clinical Focused Work: Clinical focused work aligned with RN scope of practice

Chronic Care Management and RPM Support
Support CCM and RPM programs through ongoing clinical monitoring and documentation
Review RPM data and trends, identifying potential clinical concerns
Perform lab follow ups and clinical reviews based on provider protocols
Communicate with patients regarding lab results, care plan updates, and monitoring outcomes within RN scope of practice
Escalate abnormal findings or patient concerns according to established clinical pathways
Clinical Review and Documentation
Review, analyze, and process laboratory results accurately using ECW
Ensure timely completion of assigned lab reviews within the same business day
Identify abnormal or critical lab values and initiate appropriate escalation
Maintain accurate, compliant, and timely documentation in the EMR
Systems and Workflow Management
Navigate ECW and RPM platforms to support daily clinical operations
Adhere to internal clinical guidelines, compliance standards, and turnaround time expectations
Collaborate with providers, care coordinators, and operations teams to ensure continuity of care
Licensure and Education
Experience and Qualifications:
Minimum of 2 years of clinical nursing experience
Prior experience in CCM, RPM, care coordination, or ambulatory care preferred
Experience reviewing and interpreting laboratory results
Hands-on experience with eClinicalWorks (ECW) required
Strong patient communication skills within RN scope of practice
High attention to detail and ability to work independently in a remote setting
Comfortable with technology and learning new digital health tools
Schedule Requirements:

Training schedule will follow US Eastern Time
Flexible work hours available after successful completion of training
Ability to meet daily productivity and turnaround time expectations

Compensation and Benefits
Competitive Hourly Rate: USD 6 to 7 per hour. Compensation based on experience and qualifications
Remote Work with Flexibility: While the role offers flexibility, candidates must be available to work required hours during the afternoon to evening Eastern Time to support clinical operations.after training.
