Overview

The VA Liaison for our caregiving company serves as the primary connection between veteran clients, their families, our caregiving team, and the U.S. Department of Veterans Affairs. This role is responsible for helping veterans access in-home care services through VA programs, coordinating care, and ensuring compliance with VA requirements. The ideal candidate is experienced in home care or case management and is passionate about improving quality of life for veterans. The ideal candidate will possess a strong understanding of patient care processes, and community engagement strategies, all while maintaining a lively and motivating approach to their responsibilities. Duties

  • Serve as the main point of contact for veteran clients receiving or seeking in-home caregiving services
  • Guide veterans and families through VA benefits related to home care (e.g., Homemaker/Home Health Aide, Aid & Attendance)
  • Coordinate with VA case managers, social workers, and local VA offices to secure service authorizations
  • Manage the intake and onboarding process for VA-funded clients
  • Ensure all documentation meets VA standards and company compliance requirements
  • Schedule and coordinate caregiving services in collaboration with internal staff
  • Monitor client satisfaction and adjust care plans as needed
  • Maintain accurate client records, authorizations, and service logs
  • Conduct outreach to VA facilities, clinics, and veteran organizations in the Phoenix area to build referral partnerships

Experience

Required:

  • 1+ years of experience in home care, healthcare coordination, or working with veterans
  • Knowledge of VA programs and benefits related to in-home care
  • Strong organizational and case management skills
  • Excellent communication and relationship-building abilities

Preferred:

  • Experience working directly with VA Community Care Network or similar programs
  • Familiarity with Phoenix-area VA systems and veteran resources
  • Background in caregiving, home health, or senior services
  • Associate or Bachelor’s degree in Healthcare Administration, Social Work, Business, or related field (or equivalent experience)

Pay: $54,080.00 - $62,400.00 per year
