In the SSG Fox Suicide Prevention Grant Program (SSG Fox SPGP) at WTCSB our mission is to provide resources toward community-based suicide prevention efforts to meet the needs of Veterans and their families through outreach, suicide prevention services, and connection to VA and community resources. provide a coordinated system of quality recovery-oriented care in Mental Health, Intellectual Disability, and Substance Abuse Services to the Veterans and their families of the cities of Franklin and Suffolk and the counties of Isle of Wight and Southampton. We are seeking a qualified Case Manager for our SSG Fox SPGP - Team located in Suffolk, VA


**Substance Abuse, Mental Health, or Intellectual Disability/Developmental Disability experience is required for this position. Veteran or experience working with Veterans Healthcare also required**


Responsibilities and Duties for SSG Fox SPGP Case Manager:

  • Reports to SSG Fox SPGP Operations Manager and Service Member and Family Veteran (SMVF) Regional Navigator assigning clients to WTCSB Internal Veteran Peer Support Specialists. Will also link services to outside referrals, to include the Veterans Healthcare Administration/VA
  • Responsible for consumer assessment, service linkage, coordination, development, and implementation of a case management ISP, perform careful assessment of participants and developing and monitoring case plans in coordination with a formal assessment of suicide prevention services needed, including necessary follow-up activities to ensure that the participant's needs are adequately
  • Coordinates with SSG Fox SPGP or SMVF Clinician when additional guidance in prevention programs, resources, and other family, and community issues as
  • Provide referrals to participants and related activities (such as scheduling appointments for participants) to help participants obtain needed suicide prevention services, such as medical, social, and educational assistance or other suicide prevention services to address participants' identified needs and
  • Educate participants on issues, including, but not limited to, suicide prevention services availability and participant
  • Decide how resources and services are allocated to participants on the basis of
  • Monitor service delivery through contacts with consumers receiving services, family members, service providers, and periodic site/home
  • Manage consumer records according to all Licensure and Medicaid
  • Enters participant records into Data Collection
  • Leverage trust established through relationships with program participants and support reestablishing connections with consumers having a history of reluctance to seeking care

from VA settings.

  • Perform other duties as assigned by the SSG Fox SPGP Operations Manager or Service Member, Veteran Family Regional Navigator, Director, and as approved by VA, to serve the comprehensive needs of participants for the purpose of reducing suicide

Qualifications and Skills for SSG Fox SPGP, Case Manager:

  • Must be a Veteran with a Discharge other than Dishonorable
  • BA/BS from an accredited college or university with major course experience in Psychology, Social Work, or related field
  • One year of clinical experience in a Human Services field
  • QMHP or QIDP required
  • Knowledge, skills, and abilities (KSA’s) required. Experience with CREDIBLE Behavioral Health Software is preferred, but not
  • Experience with Veteran Healthcare is the most essential factor, especially interacting with the Veteran’s Healthcare
  • Knowledge of Veteran’s Benefits Eligibility
  • Valid Driver’s License with an acceptable DMV record at the time of employment and continuously thereafter
