Veterans Case Manager

We’re looking for a Veteran Case Manager to join our team as part of Home At Last (SSVF) — a housing stabilization program that provides temporary financial assistance and services to help Veterans and Veteran families overcome homelessness and achieve lasting housing stability.

As a Veteran Case Manager, you’ll be on the frontlines, supporting Veterans with empathy, resourcefulness, and dedication. You’ll work directly with participants to navigate housing barriers and connect them to vital services, while fostering long-term self-sufficiency. Key Responsibilities:

  • Provide housing-focused case management using Housing First, Trauma-Informed Care, Motivational Interviewing, Critical Time Intervention, and Crisis De-escalation techniques.
  • Conduct participant intakes, assess needs, identify housing barriers, and develop individualized housing stability plans.
  • Connect participants to resources including benefits, employment, and community services.
  • Maintain accurate, timely documentation to meet program and grant requirements.
  • Uphold professional boundaries and foster respectful relationships with participants.
  • Assist in mediation with families and service providers as needed.
  • Specialize in areas such as Emergency Housing, Outreach, Housing Navigation, Employment, SOAR, and Rapid Resolution.
  • Other duties as assigned.

Qualifications:

  • Associate’s degree required (Bachelor’s degree preferred) in Human Services or related field.
  • 1–2 years of experience in social services, case management, or related work.
  • Familiarity with Military Culture preferred.
  • Experience working with individuals facing mental health and/or substance use challenges is a plus.
  • Knowledge of Housing First, Trauma-Informed Care, Motivational Interviewing, and de-escalation techniques preferred.
