Title: Social Service Case Manager
Position Reports to Social Service Programs Director

The Veterans/Shelter Case Manager supports the daily operations of the St. Vincent de Paul North Idaho Shelter, with a primary focus on working with veterans on housing-focused case management and supporting HMIS data entry as required. This role ensures accurate client documentation, facilitates reporting, and supports compliance with HUD, VA, IHFA, and Continuum of Care (CoC) requirements. The Case Manager works directly under the supervision of the Social Services Director. The Shelter Case Manager combines direct client engagement with operational responsibilities, including limited HMIS data entry, program compliance, and coordination of shelter services. This position works with individuals and families in Social Service Programs to promote housing stability, income growth, and connections to mainstream resources while maintaining complete
and accurate documentation.

ESSENTIAL FUNCTIONS:
1. Provide housing-focused case management in alignment with HUD and IHFA
compliance standards
2. Serve as a liaison between shelter residents, landlords, and community partners
3. Develop, implement, and maintain individualized Housing Stability Plans
4. Utilize trauma-informed, housing-first, and client-centered practices
5. Problem-solve collaboratively with staff to address resident or operational issues
6. Maintain accurate and timely case management notes and documentation
7. Attend meetings of the social service staff as needed
8. Support shelter intake and referral processes
9. Perform HMIS data entry as required, ensuring accuracy and completeness of resident
records

10. Ensure all case notes, service plans, and documentation meet compliance standards
11. Monitor data quality, completeness, and timeliness for shelter reporting purposes
12. Prepare reports and support audits or monitoring reviews as needed
13. Identify and resolve data discrepancies in collaboration with team members
14. Perform additional duties as assigned to support the mission of St. Vincent de Paul

MINIMUM QUALIFICATIONS:
 Bachelor’s degree or equivalent experience preferred
 2+ years of experience in housing programs, shelter case management, or related field
 Computer literate with working knowledge of Microsoft Office (Excel, Word,
PowerPoint, Access), Publisher
 Knowledge of office procedures and equipment
 Strong interpersonal and communication skills
 Ability to work independently and as part of a team
 Positive, customer-oriented attitude
 Demonstrated experience with case management, program compliance, or monitoring
 Ability to maintain confidentiality and professionalism in a high-demand environment

MATERIALS AND EQUIPMENT USED:

Various computer programs including HMIS, Excel, Microsoft Office, Word, printers, and skill to operate copiers, multi-line phones, and FAX machine.

PHYSICAL ACTIVITIES/MENTAL DEMANDS:

Climbing, stooping, kneeling, reaching, walking, pushing, pulling, lifting, grasping, talking, hearing, repetitive motions, light work (exerting up to 20 lbs. of force occasionally), reading detail work confidently, confidentiality, problem solving, language, stress, training, math, reasoning, verbal communications, written communication, customer contact, multiple concurrent tasks.

WORKING CONDITIONS:

Not substantially exposed to adverse environmental conditions.

Job Type: Full-time

Pay: $17.00 per hour
