Job Description
We are seeking physicians (and qualified NPs with autonomous practice authority) to join a virtual concierge primary care practice serving a selective, high-net-worth patient base. This is a part-time, flexible telehealth role designed to supplement existing clinical work. Physicians and NPs will work with both medically complex patients and patients seeking cash-pay preventive or lifestyle therapies.
Clinical Considerations
Candidates must be comfortable managing a spectrum of patients, including:
- Medically complex patients (e.g., hospice, sickle cell, or chronic pain patients requiring opioids >90 MME/day, or concurrent opioid/benzodiazepine therapy when clinically justified)
- Patients requiring atypical prescribing (e.g., narcolepsy patients needing stimulant doses above FDA-labeled maximums)
- Lifestyle/preventive patients (e.g., patients with BMI ~27 without comorbidities who request GLP-1 therapy and pay cash at the pharmacy)
All prescribing must comply with state and federal law. Our administrative and legal team will provide guidance to ensure compliance and appropriate documentation.
Position Highlights

- W-2 employment possible (with benefits such as 401(k)) depending on visit volume, licensure, and DEA registrations

Compensation
Compensation is based on visit length and medical complexity:
For Physicians (MD/DO):
- Approx. $600–$1,000/hour
- Visits ≤15 minutes: $150-$250
- Visits 15–30 minutes: $300–$500
- No visits exceed 30 minutes
- Compensation is highly dependent on experience, but these are general guidelines.
For Nurse Practitioners (NPs with autonomous practice authority):
- Approx. $250–$400/hour
- Rates vary depending on licensure coverage, DEA status, and patient complexity
Responsibilities
- Provide virtual primary care via secure telehealth platform
- Conduct initial consultations and follow-ups by synchronous video, as required by law
- Prescribe medications (including controlled substances when clinically appropriate), always in accordance with state and federal regulations
- Apply documentation and compliance strategies recommended by our team to protect both physician and patient
- Care for patients across a wide spectrum: from high-risk, medically complex cases to cash-pay preventive or lifestyle requests
- Collaborate with administrative staff who prepare charts, obtain outside records, and upload PDMP data
Requirements
- MD/DO (or NP with autonomous practice authority)
- Active DEA license(s); multi-state preferred (ideally 15+)
- Medical licenses in multiple states strongly preferred
- Willingness to learn and follow regulatory, documentation, and compliance protocols
- Malpractice insurance preferred (subsidy possible based on workload), however, company policies will be made available for qualified canidates.
Preferred Qualifications
- 15+ active state medical licenses/DEA licenses
- Internal medicine, family medicine, emergency medicine, or related primary care specialties
- Specialists considered with significant relevant experience
- Prior concierge or telehealth experience
Who Should Not Apply
We are not considering applicants who have:
- A history of disciplinary, civil, administrative, or criminal actions related to medical practice
- Prior issues such as overprescribing, patient harm, or insurance fraud
- A medical license suspension or DEA license revocation
We may consider applicants with non-medical misdemeanors, such as:
- DWI/DUI
- Disorderly conduct
- Traffic violations
Cases will be reviewed based on the nature of the violation, time elapsed, and corrective actions taken.

- Offer malpractice support and legal consultation
- Ensure clinicians can practice confidently while focusing on patient care
