The Centre for Community-Driven Response (CCDR), is a non-profit organisation. We run the Pathways telehealth telehealth nurse service to support patients and families with complex care needs.
About the role
The Pathways service is a national telehealth nurse service. It is implemented across a range of disease areas with nurses based in community/patient organisations.
We are seeking volunteer nurses to manage 3 patients per week with a commitment of at least 12 months. We are particularly looking for nurses to support clinics in palliative care, aged care, and cognitive decline. You can also choose the times you are available for patient consultations.
You will be trained and supported in your role.

Summary of duties
- Assists all patients through the healthcare system by acting as a patient advocate and assist in identifying services and how to access them.
- Develop a holistic care plan for the patient and family
- Participates in patient-centered team meetings and quality improvement initiatives.
- Engage in nurse education programs within the network.
- Manages patient care, including management of patients with multiple co-morbidities or high risk for readmission to a hospital setting.
- Participates in data collection, health outcomes reporting, and programmatic evaluation related to relevant initiatives
- Evaluates clinical care, utilisation of resources, and development of new clinical tools, forms, and procedures
Education and experience
- Graduation from an accredited university with a Bachelor of Nursing Degree
- Expertise and/or excellent knowledge of the local health system
- Registration with the relevant accrediting body
- Proficient computer skills, including Microsoft Office
- 5 years minimum experience in the local clinical setting
- Self-disciplined, energetic, passionate, innovative
- A team player that can follow a system and protocol to achieve a common goal
- Highly organized and well-developed oral and written communication skills
- Demonstrates sound judgment, decision-making and problem-solving skills
- Able to maintain confidentiality with all aspects of information in accordance with practice and local legislation/regulations
