Ware MDS Care Plan Coordinator

Join our compassionate and dedicated team of caregivers at The Ware at Park Central. We are looking for an MDS Care Plan Coordinator who works in collaboration with the DON. In this role, you’ll help ensure residents receive personalized, high-quality care by leading assessments, developing care plans, and coordinating with our interdisciplinary team in alignment with Baptist Community Services' core values and regulatory standards. This is an hourly, non-exempt position.

EDUCATION: Texas Nursing License, RN or LVN as defined above. MDS experience in SNU or LTC setting required. Supervisory experience preferred. RUG certification preferred but may be obtained within the first 30 days of employment. RAC certification preferred by may be obtained within the first 30 days of employment. Cardiopulmonary Resuscitation (CPR) Certification required.

Essentials of Job:

  • RN MDS/Care Plan Coordinator –
    • Schedules resident assessments and care plan initiation/revision. (Constantly)
    • Coordinates and participates in the completion of the Minimum Data Set (MDS). (Constantly)
    • Coordinates and participates in the completion of the Care Area Assessments as defined by the RAI Manual. (Constantly)
    • Initiates, coordinates, and facilitates the Interdisciplinary Care Planning process including meeting with residents and families. (Constantly)
    • Evaluates resident’s response to plan of care making changes as needed and in collaboration with the interdisciplinary team. (Constantly)
    • Monitors clinical documentation and provides on-going education related to data collection and resident care. (Constantly)
    • Analyzes and evaluates quality measure and 5-star data reporting findings to the Director of Nursing and Performance Improvement (PI) Committee. (Frequently)
  • LVN MDS/Care Plan Coordinator -
    • Facilitates and participates in the completion of the MDS. (Constantly)
    • Facilitates the Interdisciplinary Care Planning process including meeting with residents and families. (Constantly)
    • Evaluates the resident’s response to the plan of care making recommendations to the interdisciplinary team for modification. (Constantly)
  • Plans, organizes, and conducts training in conjunction with the Clinical Education department related to resident assessment and data collection. (Frequently)
  • Reviews Ware Policy and Procedures that guides the Resident Assessment and Care Plan process. (Occasionally)
  • Perform direct emergency medical care to customers and staff. (Occasionally)
  • Take on-call rotation with nurse managers (Occasionally)
  • Advocate equally for all residents. (Constantly)
  • Perform other duties as needed and assigned. (Occasionally)
  • Must be able to commit the time necessary from an attendance and punctuality standpoint to accomplish the above specified Essentials of Job. (Constantly)

PHYSICAL/SPECIAL DEMANDS:
1. Must have the ability to understand meanings of words and respond effectively.
2. Prepare and assess reports.
3. Communicate interpersonally as well as utilizing telephone, computer, and other electronic devices.
4. Ability to hear and speak effectively on phone and use computer and other electronic devices.
5. Remain in a stationary position for greater than 50% of the time.
6. Move throughout the facility.
7. Position self to reach from floor level to six feet from the floor.
