Weekend-Only Hospice Triage Nurse

Overview

Badger Hospice, LLC is seeking a skilled and compassionate After Hours & Weekend Triage Nurse to support patients, families, and care teams during evenings, nights, weekends, and holidays. This role is essential to ensuring continuity of high-quality hospice care outside of standard business hours.

The ideal candidate is a confident clinician with strong assessment skills, sound clinical judgment, and a commitment to the hospice philosophy of comfort, dignity, and patient-centered care. In this role, you will serve as the primary clinical point of contact after hours, providing telephone triage, symptom management guidance, and clinical support to patients, caregivers, and staff.

This position focuses on remote triage and coordination of care rather than in-person patient visits.

Responsibilities

  • Provide after-hours telephone triage and clinical support to hospice patients, families, and care teams.
  • Assess patient symptoms and concerns remotely and determine appropriate interventions.
  • Provide guidance to caregivers regarding symptom management, medication use, and comfort measures.
  • Serve as the primary clinical contact after hours, responding to patient, family, and staff needs in a timely and compassionate manner.
  • Coordinate care with on-call field staff when in-person visits are needed.
  • Review and process referrals and admissions during after-hours coverage when applicable.
  • Communicate changes in patient condition to the attending physician, medical director, Clinical Director, and interdisciplinary team.
  • Provide education and emotional support to patients and families regarding disease progression, symptom management, and end-of-life expectations.
  • Complete accurate and timely documentation in accordance with hospice regulations and organizational standards.
  • Participate in quality improvement efforts and support continuity of care through follow-up communication with the care team.

Schedule & Coverage Requirements

This is a set-schedule, every-weekend position.

  • Coverage is Friday at 5:00 PM through Monday at 8:00 AM
  • Includes on-call triage responsibilities during this timeframe
  • Holidays may be required based on organizational needs.
  • Schedule is consistent and not rotating.

Qualifications

  • Current WI Registered Nurse (RN) license in good standing.
  • Minimum of two years of nursing experience preferred, with background in hospice, home health, med-surg, or critical care.
  • Strong knowledge of symptom management, end-of-life care principles, and hospice regulations.
  • Excellent clinical judgment, assessment, and documentation skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Strong communication skills to support patients, families, and staff during emotionally sensitive situations.
  • Experience with telephone triage or remote clinical support preferred.
