Wound Care NP/PA

Job Overview
We are seeking a dedicated and skilled Wound Care Nurse Practitioner to join our healthcare team. The ideal candidate will possess a strong background in wound care management and demonstrate a commitment to providing high-quality patient care. (training also available) This role involves assessing, diagnosing, and treating patients with various types of wounds, ensuring optimal healing and recovery through evidence-based practices.

Duties

• Conduct comprehensive assessments of patients with wounds, including history taking and physical examinations.
• Develop individualized care plans based on assessment findings and patient needs.
• Perform wound debridement, dressing changes, and other necessary procedures to promote healing.
• Collaborate with interdisciplinary teams to coordinate care for patients in various settings, including home care and senior care facilities.
• Document all patient interactions accurately in medical records using electronic health record systems such as Epic.
• Educate patients and their families on wound care management, prevention strategies, and lifestyle modifications.
• Stay current with advancements in wound care techniques, technologies, and best practices.

Skills

• Proficiency in ICD-10 coding for accurate medical documentation.
• Strong foundation in medical-surgical nursing principles.
• Experience with home care services and senior care environments.
• Knowledge of sonography techniques related to wound assessment.
• Familiarity with gastroenterology-related wound issues is a plus.
• Competence in medical imaging as it pertains to wound evaluation.
• Excellent communication skills for effective patient education and collaboration with healthcare teams.
• Strong organizational skills to manage multiple cases efficiently while maintaining attention to detail.

Pay: $100,000.00 - $150,000.00 per year
