Job Summary
We are seeking a dedicated and highly skilled Wound Care Nurse Practitioner to join our dynamic healthcare team. Responsibilities
- Conduct thorough wound assessments, including evaluating wound type, size, depth, and signs of infection or complications.
- Develop and implement individualized wound care plans utilizing evidence-based practices and advanced techniques such as debridement, dressings, and skin grafts.
- Administer specialized treatments including Botulinum toxin injections for specific conditions and esthetic laser therapies when applicable.
- Manage complex cases involving diabetic foot ulcers, pressure injuries, surgical wounds, and vascular wounds across various settings.
- Collaborate with multidisciplinary teams—including physicians, physical therapists, dietitians, and case managers—to optimize patient recovery and prevent recurrence.
- Educate patients and caregivers on wound care procedures, infection prevention, nutrition, and lifestyle modifications to promote healing.
- Maintain accurate medical documentation using EMR (Electronic Medical Records) systems such as Epic or eClinicalWorks; ensure compliance with HIPAA regulations.
- Stay current with advancements in wound management techniques, ICD coding standards (ICD-10), CPT coding for procedures, and relevant clinical research to enhance practice quality.
Requirements
- Valid Nurse Practitioner license with current certification in wound care or related specialty preferred.
- Proven experience in acute care settings such as hospitals (Level I or II trauma centers), ICU units, or emergency rooms.
- Strong background in critical care, infectious disease management, diabetes management (including limb salvage), and post-operative wound care.
- Proficiency with EMR/EHR systems like Epic or Cerner; experience with Athenahealth is a plus.
- Knowledge of advanced procedures such as suturing, catheterization, IV infusion therapy, sterile technique, and venipuncture.
- Familiarity with telehealth platforms for remote patient monitoring and consultations.
- Ability to work effectively with diverse populations including pediatrics (Pediatrics), geriatrics (Memory Care), hospice patients (Palliative Medicine), individuals with developmental disabilities or chronic conditions.
- Excellent patient assessment skills combined with physiology knowledge to evaluate complex cases quickly and accurately.
- Strong communication skills to educate patients on wound management strategies effectively while ensuring compliance with healthcare regulations such as HIPAA.
