Wound Care Nurse Practitioner

Job Overview
We are seeking a dedicated and highly skilled Wound Care Nurse Practitioner to join our dynamic healthcare team. Your expertise will support patients across various settings, including outpatient clinics, post-acute care facilities, and inpatient units. Responsibilities

  • Conduct comprehensive patient assessments focusing on wound evaluation, including measuring, photographing, and documenting wound characteristics using electronic health record (EHR) systems such as Epic or eClinicalWorks.
  • Develop personalized wound care plans incorporating advanced treatments such as debridement, dressings, skin grafts, and Botulinum toxin treatment when appropriate.
  • Perform procedures including suturing, catheterization, IV insertion, and sterile dressing changes following aseptic techniques.
  • Manage complex cases involving diabetic foot ulcers, pressure injuries, surgical wounds, and infectious wounds with a focus on infection control and healing optimization.
  • Maintain accurate medical documentation adhering to HIPAA regulations and CPT coding standards for billing purposes.

Skills

  • Proven experience with electronic health record (EHR) systems such as Epic or Athenahealth for documentation and clinical workflows.
  • Strong knowledge of physiology, anatomy, and diagnostic evaluation related to wound healing processes.
  • Skilled in performing advanced procedures including suturing, phlebotomy, sterile dressing changes, and IV infusion.
  • Familiarity with complex medical conditions such as diabetes management, infectious disease care, acute pain management, and hospice & palliative medicine.
  • Experience working in diverse settings such as hospital medicine units—including Level I Trauma Centers—and outpatient clinics specializing in geriatrics or memory care.
  • Ability to perform patient assessments involving vital signs, physical examinations, and triage for urgent or emergent cases.
  • Knowledge of CPT coding for billing procedures related to wound care services; understanding of ICD-10 coding for diagnoses.
  • Competence in infection control practices including aseptic technique and sterile processing protocols.
  • Excellent communication skills for patient education and interdisciplinary collaboration across specialties like dermatology, internal medicine, pediatrics, or behavioral health.
