We are seeking a highly skilled and compassionate Wound Care Nurse Practitioner or Physician Assistant to join our healthcare team. As a Wound Care Provider, you will be responsible for providing specialized wound care assessment, treatment, and management to patients with acute and chronic wounds in long-term care facilities. You will work collaboratively with physicians, nurses, and other healthcare professionals to develop individualized treatment plans and ensure optimal wound healing outcomes.
Responsibilities:
- Perform comprehensive wound assessments, including wound dimensions, characteristics, tissue type, presence of infection, and pain levels.
- Collaborate with the interdisciplinary team to develop and implement individualized wound care treatment plans based on the patient's condition, medical history, and specific needs.
- Provide advanced wound care interventions, including wound cleansing, debridement, dressing application, and wound closure techniques.
- Prescribe appropriate medications, including topical agents, antibiotics, and pain management medications, as needed.
- Order and interpret diagnostic tests, such as wound cultures, imaging studies, and laboratory tests, to guide treatment decisions and assess wound healing progress.
- Educate patients, families, and caregivers on wound care techniques, preventive measures, and the importance of adherence to the treatment plan.
- Monitor and evaluate the progress of wound healing, documenting findings and adjusting treatment plans as necessary.
- Collaborate with other healthcare providers, such as surgeons, vascular specialists, and physical therapists, to ensure comprehensive and coordinated care for patients with complex wounds.
- Stay updated with the latest advancements in wound care research, technologies, and evidence-based practices.
- Participate in quality improvement initiatives, audits, and research activities related to wound care management.
- Maintain accurate and thorough documentation of wound assessments, treatment interventions, and patient outcomes in electronic medical records.
- Adhere to legal and ethical standards of nursing practice and ensure compliance with regulatory requirements.
