Job Description

The Youth Crisis Outreach Team (YCOT) program provides crisis intervention and assessment, intensive case management, relapse prevention, and care coordination services to youth in crisis. This program partners with the Mobile Crisis Outreach Team and responds to crisis 24/7 in schools, emergency rooms, homes and clinics. The core components of a crisis continuum are somewhere to call, someone to respond, and a place to go. The Bluebonnet Trails Youth Outreach Specialist has a key role in providing quick, in person response to those in crisis, and continuing to provide transitional services for up to 90 days to each youth and family.

Our Vision is Healthy and Fulfilled Lives, and YCOT offers an interdisciplinary team of dedicated clinicians supporting youth and families at the time of crisis, and for months following the crisis in order to ensure stabilization and linkage to longer term community supports. Serving the whole family system, the YCOT team consists of Youth Outreach Specialists, Certified Family Partners, Certified Peer Support Specialists, Therapists, and linkage to other services and resources as needed. The Youth Outreach Specialist provides support and assistance with the completion of obtaining vital documents, benefits completion, and employment services as needed.

Our Mission is to continuously improve the health and independence of the persons we serve, and the Youth Outreach Specialist provides timely access to care at the time of crisis, using evidenced based intervention and evidence-based practices to reduce utilization of higher level of care options, and reduce intersection with the criminal justice system.

Critical Impact:

The YCOT team provides immediate connection to crisis intervention and a consistent team to provide crisis follow up and transitional services after the initial crisis has been resolved. Through intensive supports, provided from a daily to weekly basis, the Youth Outreach Specialist collaborates with the youth and family on goals, creating a plan of action to achieve those goals. The Youth Outreach Specialist is the primary point of contact for the youth and family, collaborating with internal and external teams, including the DFPS, schools, law enforcement, primary care, and hospitals.

Key Responsibilities:

  • Provides person-centered care, including crisis intervention, routine and intensive case management, supported housing and employment, care coordination, and assistance with benefits completion
  • Participates in consistent 360 degree feedback with direct supervisor, including collaboration on individualized support, goal setting, and coaching
  • Builds relationships with colleagues, learning about options and resources at BTCS and in the community
  • Responsive to needs to a changing and growing program, maintaining flexibility as the program develops and continues to improve
  • As a Crisis Clinician, functions as a member of the Mobile Crisis Outreach Team, including after hours on call rotation

Location: Williamson and Burnet Counties

Hourly Salary Range: $25.23 – $37.48

Differential Detail:

  • Up to $3.50/hour – Previous crisis experience
  • Up to $2.00/hour- Previous experience in similar role with a community center in Texas
  • Up to $4.00/hour – Tenure with BTCS
  • $.75/hour – Bilingual
  • $1.00/hour – Masters Degree in Human Services
  • $1.00/hour – Advanced Licensure
  • $.50/hour – LCDC Licensure

Plus Shift Differentials

  • $30.00/day – Rotating Weekly On-Call Stipend
