Your dedication will empower clients to maintain independence, safety, and dignity while receiving the highest standard of home and community-based care.Duties<ul><li>Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support</li><li>Monitor patient health by taking vital signs, observing symptoms, and reporting changes to healthcare teams</li><li>Administer medications accurately following care plans and HIPAA privacy regulations</li><li>Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking</li><li>Utilize specialized equipment such as Hoyer lifts or other assistive devices to ensure safe patient transfers and mobility</li><li>Maintain a clean and safe environment by performing light housekeeping tasks like cleaning, laundry, and organizing living spaces</li><li>Support clients with memory care needs through engaging activities and personalized routines for dementia or Alzheimer's care</li><li>Document all care activities meticulously in compliance with healthcare standards and care plans</li><li>Collaborate with healthcare professionals, social workers, families, and other caregivers to deliver coordinated support</li><li>Assist with transportation for medical appointments or community outings when required</li></ul>Qualifications<ul><li>Proven experience in home health, assisted living, nursing homes, or group home settings is highly preferred</li><li>Knowledge of patient observation techniques, medication administration, and vital signs monitoring</li><li>Certification in CPR and First Aid is required; additional certifications such as HIPAA training are advantageous</li><li>Experience working with individuals with developmental disabilities, autism spectrum disorder, dementia or Alzheimer&rsquo;s disease is a plus</li><li>Ability to perform heavy lifting safely and operate patient transfer equipment like Hoyer lifts</li><li>Strong communication skills to effectively interact with clients, families, and healthcare teams</li><li>Compassionate attitude with a commitment to providing respectful and dignified care</li><li>Flexibility to work various shifts including mornings, evenings, weekends or overnight as needed</li></ul>Join us in making a meaningful difference!
