In-Home Care Provider

Join our compassionate team as an In-Home Care Provider, where you will play a vital role in enhancing the quality of life for individuals needing personalized care and support within their own homes. Duties

  • Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Monitor patient health by taking vital signs, observing symptoms, and reporting changes to healthcare teams
  • Administer medications accurately following care plans and HIPAA privacy regulations
  • Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking
  • Utilize specialized equipment such as Hoyer lifts or other assistive devices to ensure safe patient transfers and mobility
  • Maintain a clean and safe environment by performing light housekeeping tasks like cleaning, laundry, and organizing living spaces
  • Support clients with memory care needs through engaging activities and personalized routines for dementia or Alzheimer's care
  • Document all care activities meticulously in compliance with healthcare standards and care plans
  • Collaborate with healthcare professionals, social workers, families, and other caregivers to deliver coordinated support
  • Assist with transportation for medical appointments or community outings when required

Qualifications

  • Proven experience in home health, assisted living, nursing homes, or group home settings is highly preferred
  • Knowledge of patient observation techniques, medication administration, and vital signs monitoring
  • Certification in CPR and First Aid is required; additional certifications such as HIPAA training are advantageous
  • Experience working with individuals with developmental disabilities, autism spectrum disorder, dementia or Alzheimer’s disease is a plus
  • Ability to perform heavy lifting safely and operate patient transfer equipment like Hoyer lifts
  • Strong communication skills to effectively interact with clients, families, and healthcare teams
  • Compassionate attitude with a commitment to providing respectful and dignified care
  • Flexibility to work various shifts including mornings, evenings, weekends or overnight as needed
