Responsibilities:
<ul><li>
Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).</li><li>
Completes regular re-evaluation of patient needs.</li><li>
Initiates that Plan of care and make necessary revisions as needed.</li><li>
Develops care plans, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.</li><li>
Initiates appropriate preventive and rehabilitative nursing procedures and administers medications and treatments as ordered by the medical provider.</li><li>
Counsels and provides education to the patient and family related to the patient's care needs.</li><li>
Case Management</li><li>
Prepares and updates clinical notes</li><li>
Communicates with the Medical Provider regarding the patient's needs and reports any changes in the patient's health. Review physician's orders as necessary and facilitate changes in care plan.</li><li>
Provides discharge planning as part of the care plan.</li><li>
Participates in an-call duties as defined by company policy</li></ul>

Position Qualifications:
<ul><li>
Graduate of an approved nursing program/school</li><li>
Current State of MI Licensed RN with one to two years of recent acute care experience.</li><li>
Must possess a valid driver's license and acceptable driving record</li></ul></div>
